The form asks for your business name, full name, and phone number. If a Google Places key is configured, choosing a suggestion can also store a place id and the address Google returns. If a future version of the form asks about revenue or a Google Business Profile, those answers are collected too.
We use this information to call you within 5 minutes, check whether your 30-mile radius is still open, and decide whether the Match Method is a fit. We do not sell or rent your personal information.
